Here’s how to get started:
- Access our Patient Portal by clicking the button above.
- Click Create an Account.
SIGNING UP AS THE PATIENT
1. If you are the patient...
- Select Patient.
- Enter your Date of Birth.
- Enter the email you provided when you scheduled your first appointment.
2. Check your email for a confirmation link. The subject line will read: “Next steps: Patient Portal registration with Providence Medical Partners.” In that email, click Create My Account.
3. Share a bit more about yourself. The portal will ask you to enter basic details, like your name, gender, phone number and communication preferences. We recommend consenting to texts and calls. That way, it’s easy for your provider to connect with you.
4. Select your provider and their location. This helps us connect your account to the right clinic.
5. Create your new password. You will use this password each time you access the portal, so choose something secure yet memorable!
6. The portal will ask you to verify your identity through one of three channels. You can receive a temporary passcode through:
- Email: If you choose this option, the portal will send a temporary passcode to the email address you entered previously.
- Phone call: If you choose this option, you will receive an automated call at the phone number you provide. During the automated call, a voice will read off your temporary passcode. Once you’ve written down the code, you may hang up.
- Text message: If you choose this option, you will receive an automated text at the phone number you provide. This text will contain your temporary passcode.
7. Your registration process is complete! Click Continue to enter your portal.
SIGNING UP AS A FAMILY MEMBER/CAREGIVER
1. If you are creating an account to manage the care of another person...
- Select Caregiver or Family Member.
- Enter your date of birth (not the patient’s). This is to verify you are an adult.

2. Enter your personal information. Remember – these should be your personal responses. You don’t have to enter any information on behalf of the patient yet.
3. Check your email for a confirmation link. The subject line will read: “Next steps: Patient Portal registration with Providence Medical Partners.” In the email, click Create My Account.
4. Enter the patient’s information

5. Verify your identity. The patient (not you) will receive a temporary passcode. You can choose how this passcode should be sent (email, call or text). However, the message will go to the email or phone number on the patient’s file. Once you’ve selected your preferred method, click Send Code.
Enter the temporary passcode the patient received, then click Continue.
6. Create your new password. You will use this password each time you access the portal, so choose something secure yet memorable!
7. Your registration process is complete! Click Continue to enter the portal.
